Accidents, errors, adverse events, near misses, safety incidents whatever you call them they can happen at any time. Learning from these incidents is vitally important as it can reduce the chance of them happening again. In order to react appropriately to an incident it needs properly investigating in an open blame-free fashion. This lecture uses real life incidents to demonstrate to the audience how they can investigate safety incidents in their practice using a number of different, simple, practical incident analysis techniques. This type of technique will be useful to those looking to perform clinical audits and interested in improving safety in their practice.
